How to apply for residential care

After you had done an assessment from ACAT and received a approval letter for the entry of an aged care home, please follow the 5 Steps pathway to enter an aged care home and manage your services:

  1. Work out the costs
  2. Find an aged care home
  3. Apply to an aged care home and accept an offer
  4. Enter into agreements with your aged care home
  5. Manage your care and services

The forms can be downloaded by clicking on the links below. These forms are issued by the Commonwealth Government, and the latest versions can also be downloaded from the MyAgedCare and Department of Social Services website.

To apply to Elderly Chinese Home please provide the following:

  • A Copy of the application Form which can be downloaded from our website www.echinc.com.au
  • A letter of eligibility to enter a Residential Aged Care Home An assessment conducted by the Aged Care Assessment Team and you will be provided with a letter of eligibility to enter a Residential Aged Care Home.
  • Statement of Resident Aged Care Fees for providers following your completion of the form called the Permanent Residential Aged Care – Request for a Combined Assets and Income Assessment (SA457) to provide your income and asset information. You should return the form to the Department of Human Services or the Department of Veterans’ Affairs as per the instructions on the form.

To apply for care, complete the Application for Respite Care or Permanent Entry to Aged Care Homes form. This may be photocopied and used for applications at different care facilities. The application form must be accompanied by a copy of a current aged care assessment form, and also your asset assessment if it is available.

The Statement of Resident Aged Care Fees for providers can be obtained through completing the Request for Asset Assessment form, and sending it to Centrelink, Canberra or Veterans’ Affairs, whichever is your pension provider. If you need assistance in completing this form, you can download the information booklet.

If you want to deal with the Department of Health and Ageing on behalf of someone else, you will need to complete the Appointment of a Representative Form. This is in addition to any form you may have already completed for Centrelink.

Respite Care

If you have a current Aged Care Assessment recommending low or high care residential respite care, you may have up to 63 days of respite care in each financial year, with allowable extensions of 21 days at a time if necessary.

When receiving residential respite care, you will be asked to pay a basic daily care fee. The fee for residential respite care is based upon the current pension rate and is affordable.

Access to residential respite care

You will need an ACAT assessment for respite in an aged care home. The availability of services varies from region to region, and the assessment will determine your needs in line with what help is available in your area.

If you need emergency respite care, phone your local Commonwealth Respite and Carelink Centre on 1800 052 222 during business hours or 1800 059 059 outside business hours.

At ECH, we offer Interim Respite care only. This means we will not be able to take scheduled, planned respite care needs and that we will be able to assist carers who are in need of rest on and off through the year, when a bed is available for a short period of time.
