如何申請入住安老之家：
在你經ACAT審核合格，並取得一封批準申請入住老年護理機構的信後，請按照以下5個步驟進入老年護理機構：
這些表格是由聯邦政府發布的，可以通過點擊下面的鏈接下載。最新版本也可以從MyAgedCare和社會服務部的網站下載。
申請入住安老之家需提供：
- 一份申請表 ，可於安老之家網址下載。 www.echinc.com.au
- 一封批準申請入住老年護理機構的信。 經ACAT審核合格後，你將獲得一封批準申請入住老年護理機構的信。
- Statement of Resident Aged Care Fees for providers following your completion of the form called the Permanent Residential Aged Care – Request for a Combined Assets and Income Assessment (SA457) to provide your income and asset information. You should return the form to the Department of Human Services or the Department of Veterans’ Affairs as per the instructions on the form.
申請入住老年護理機構，請填寫 《暫住或永久居住的申請表》這可能會複印並用於不同的護理機構。申請表必須附上一份老年護理評估表的副本及資產評估（如有）。
《居民養老費用報表》需通過填寫 《資產評估申請表》, and sending it to Centrelink, Canberra or Veterans’ Affairs, whichever is your pension provider. If you need assistance in completing this form, you can download the 小冊子.

臨時住宿護理
如果你目前的老年護理評估建議提供低度或高度的臨時住宿護理，你在每個財政年度最多可以獲得63天的臨時護理，如有必要，將允許每次延長21天。
在接受臨時住宿護理期間，你將被要求支付每日的基本護理費用。臨時住宿護理的費用是基於目前的養老金標準。
申請臨時住宿護理
在申請臨時住宿護理前，你需要進行ACAT評估。各個地區提供的服務都不同，評估將根據你的需求和你所在的地區提供幫助。
